HOME CARE SERVICES
chcs-NEXUS provides a variety of Homecare services. Based on the needs of the individual, these services fall into several categories of home care. These include services for individuals with special care situations caused by medical conditions such as diabetes or heart failure or injuries from an accident.
CAREGIVER SERVICES
The types of caregivers who are available at chcs-NEXUS to provide In Homecare Services include RN’s, LPN’s, CNA’s, HHA’s, Companions and Homemakers. Services that referred caregivers can provide include simple Homemaking activities such as Meal Preparation, Light Housekeeping, Transportation, Medication reminders, Prescription Pick-up or simple Fellowship.
